Review failed runs

Find the cause#

1. Open Runs → History and search for the task or automation ID.
2. Expand Details and locate the failed process or workflow step.
3. Open the original task transcript for tool input, output, approval, and error context.
4. Check whether an external action completed despite the reported failure.
5. Fix the exact credential, permission, input, policy, network, or Runtime issue before retrying.

Expected result#

You can name the failed layer and the last confirmed effect. Preserve the run and transcript as evidence until recovery is complete. A green replacement run does not prove the first attempt had no side effect.
